The Clinical records department is available between 9am and 3pm, Monday to Friday.

Information from your records, or the records of a dependant, can be requested from the Clinical records department. This will include documentation from Auckland City Hospital, Starship Hospital and Greenlane Clinical Centre.
Complete the request form and provide the requested identification. Identification requirements are on page three of the form.

Please note: If the request is for the clinical record of a family member who is not a dependant (16 years or under) then that person's consent is required in writing.
