Needs assessment service co-ordination is about planning your care based on your goals and support needs.
When assessing your support needs, we use a range of tools from within the InterRAI series:
- contact assessment
- HomeCARE assessment
- palliative assessment
- mental health assessment.
Following this, we may be able to allocate and co-ordinate support services to meet the needs we identify in the assessment.
We share information with everyone who helps care for you. We work collaboratively with you and your whānau. We can:
- help you access community care and support services
- talk with health and community care providers
- plan what services you might need in the future
- provide education and support for you and your carer.
NASC works with people who have:
- older people's needs
- disability needs
- mental health needs.
