Case Managers

Before or during a stay in hospital, a patient will have contact with many different departments and staff members. The Taranaki DHB aims to make sure you and your family receive full and consistent information. Case Managers are there for you and your family to ask about any part of your care. If you need advice regarding community services or help at home after you leave hospital, they can assist with this too.

Case Managers make sure your care progresses smoothly across all departments in the hospital and in the community. They work to improve the hospital's services to patients overall and coordinate communication between the patient, family members, GPs, ACC and health care agencies (normal Privacy Act considerations apply).
