Ensuring service integration of new models of care across all healthcare providers.

The Community Forum (click here to view the members) will:

Ensure community involvement in health service planning.
Increase community understanding of health issues.
Provide community input to Clinical Forum discussions on patient pathways.
Provide the link between their communities and the DHB.
Help ensure that their communities understand how to access health services.

South Taranaki Clinical Forum

The Clinical Forum (click here to view the members) ensures that doctors, nurses and other health care professionals working for South Taranaki people, have input into health service provision by:

Providing clinical advice and expertise to the South Taranaki Service Integration Steering Group.
Exploring different views of all clinicians working in South Taranaki with a view to reaching mutual understanding of issues and agreed patient pathways.
Designing monitoring framework to assess compliance with new patient pathways and the impact of the pathways on the overall system effectiveness.
